All
students participating in Co-Curricular Athletics at Ponaganset High
School and Middle School must submit all off the following forms to the
Director of Athletics to competing in practice or games.
Parent Permission: The
Parent Permission form is an acknowledgement that parental consent to
participate has been granted. This form is required for every season
that an athlete competes. Students and their parents or legal
guardian must also sign permissions and waivers for the district and RI
Interscholastic League. The required forms are included as links at the
end of the page.
Physical Form: A
current physical exam is required for all athletes. Physicals are
valid for one calendar year from the date of issue. The Ponaganset form
is preferred, but not mandatory. Doctors may chose to issue their own
Physical form, which is accepted by the Athletic Department.
Proof of Personal Insurance Coverage: Each
student must have medical insurance to compete in athletics. The Proof
of Insurance form annually certifies that the athlete has medical
coverage through a specific company, including the policy number. If a
student is not covered by insurance through their parent/guardian, they
must obtain the School Insurance to participate in athletics.
School Insurance: School
Insurance is offered to students and student-athletes as a primary
insurance policy or to supplement their primary medical insurance.
Please return these forms directly to the Athletic Director. If
a student is not covered by insurance through their parent/guardian,
they must obtain the School Insurance to participate in athletics.
Concussion Information Pamphlet: This
pamphlet must be signed by the parent and athlete annually to
acknowledge they have received information regarding the effects of
concussions.
RIIL/RIPCOA Assumption of Risk: The Rhode
Island Interscholastic League and RIPCOA mandate that each athlete
complete and submit this Assumption of Risk form, which certifies a
student's home address and other information. This form must be
notarized once in the athlete's high school career.
